• Family Intake Forms - DUE Aug. 9, 2024, or at the time of mid-year enrollment

  • Vaccination records will be checked in the state database by the director prior to the start of school. Families are responsible for making sure child vaccines are up to date with the pediatrician and in the Georgia state database.

  • Allergy plan form file:///D:/MTLPS_Pics/Return%20Montage/ECP-FAAP_Plan_English-05.12.pdf (or pediatrician-specific form is acceptable)

  • Any other medical plans need to be official from the pediatrician. Written medical directives from parents will not be accepted.
